A secondary school girl come to your pharmacy asking you some preparation for pimple treatment. She had some close comedone in her forehead and 4 minor outbreak at the T zone area. Her friend recommended her this brand called PanOxy. After looking through the counter, she found that there are three types, namely, 2.5, 5 and 10. She is asking you which one is suitable for her. Besides, she wanted you to recommend her a antibacterial facial wash.
1. What is the active ingredient in PanOxy and what is the different in the three types of PanOxy?

Doxycycline:
-1 capsule is to be taken every morning for 1 month.
-This is an antibiotic and it is to be taken at regular intervals. Do not skip any doses. Continue to take it even when you feel better. You must complete the entire course of antibiotic. If you don't, the infection will not clear completely.
-Take the capsule together with food or immediately after a meal, with a full glass of water in a sitting or standing position. Do not lie down within half an hour after swallowing it.
-Common side effects: diarrhoea, nausea, vomiting and stomach discomfort. These side effects are usually temporary. If you have diarrhoea, drink lots of water to replace the fluids lost. However, if you have severe diarrhoea or diarrhoea with severe stomach cramps or bloody stools, alert your doctor.
-If you develop rashes, breathlessness, swollen mouth or eyes, stop the medicine and inform your doctor quickly. These could be signs of an allergic reaction.
-While taking this medicine, may cause your skin to be more sensitive to sunlight. Avoid direct and long hours of exposure to the sun and other forms of ultraviolet (UV) light like tanning beds or sun lamps.
-Avoid alcohol.
-Do not take capsule with dairy products such as milk, cheese, yoghurt or ice-cream. These could reduce the effectiveness. Take the capsule at least 2 hours before or after you have taken any dairy products.
Differin Gel
-The gel is to be applied on affected area every night for 1 month.
-Avoid contact with eyes, lips, angles of the nose & mucous membranes. Do not apply to cuts, abrasions and severe acne. Minimize exposure to excessive sunlight, including sunlamps.
-Some side effects: scaling, dryness, burning
-Wash your hands after applying the gel to affected area.
Clindamycin Lotion
-Lotion is to be applied to affected area twice daily for 1 month.
-Wash your hands after applying the lotion.
-Some side effects: Skin irritation & dryness, stinging of the eye, skin oiliness.
-If you develop rashes, breathlessness, swollen mouth or eyes, stop the medicine and inform your doctor quickly. These could be signs of an allergic reaction.

-Use on the skin only. Avoid getting it in your eyes, nose, or mouth, on highly inflamed or damaged skin, or on your lips. If you get them in your eyes, immediately wash them out with cool tap water.
-Do not apply acne treatment product to raw or irritated skin, including sunburns, or to open wounds.
-Talk with your doctor before you use any other medicines, special cleansers, aftershave, or cosmetics on your skin.
-Common side effects: Dryness; feeling of warmth, irritation, itching, mild peeling, redness.

1.This is Apo-Doxy(Doxycycline).It is for your bacterial infection.
2.Take 1 tablet every morning for 1 month.
3.Take tablet with food.
4.Take dose with full glass of water and remain upright for at least 30 minutes.You do not have to stand up for 30 minutes, just prevent from lying down will do.
5.Remember to take dose in an upright position as well.
6.Avoid taking with dairy products like milk.
7.It may cause photosensitivity, so avoid under the sun or going for sun tanning.Apply sun block lotion whenever you need to go out.
8.Do not take if pregnant or trying to become pregnant.
9.Do not take if wisdom tooth is growing. (tooth development, check with doctor)
10.Some side effects include nausea, vomitting and diarrhoea.
11.To prevent gastric side effects like nausea, vomitting and diarrhoea, take tablet with food or after food.
12.If you experience diarrhoea, try to replenish fluid loss by drinking water and taking more fruits and vegetables. However, if diarrhoea or any side effects are too severe, consult doctor immediately.
13.Avoid alcohol during course of medication.
14.As this medication is an antibiotics, remember to complete the whole course of medication, even if you feel better.

Ans: Salicylic Acid
Salicylic acid is a mild comedolytic agent and surface keratolytic agent.It has a potent keratolytic action and a slight antiseptic action when applied topically. It softens and destroys the stratum corneum by increasing endogenous hydration which causes the horny layer of the skin to swell, soften, and then desquamate.
6. What are the other advice can you give her when she is using acne treatment product?
Ans:
Firstly, I would recommend her the PanOxyl 2.5 first. If she feels that her condition doesnt improve,I would then recommend the PanOxyl 5 or PanOxyl 10.
1.Apply thin layer to affected area once daily.
2.Avoid eyes, nose or mucous membranes.
3.Avoid going under the sun as it might cause skin sensitivity. Avoid sun tanning etc.
4.Avoid using on inflammed or broken skin.
5.Consult doctor if skin alergies occur(rashes).
5.Other advises would be:
-It can be triggered by high levels of humidity and sweating. So try to wipe sweat whenever possible.
-Try not to use hands to touch affected areas.
-Drink more water, take more fruits and vegetables.

the same condition you have.
- If you need to have any type of surgery, tell
the surgeon ahead of time that you are taking
doxycycline. You may need to stop using the

- Throw away any unused drug when it expires or
when it is no longer needed. Do not take any
of it after the expiration date printed on the
label. It will cause damage to kidneys.
- It can cause diarrhea, which may be a sign of
a new infection. If you have diarrhea that is
watery or has blood in it, call your doctor.
Do not use any medicine to stop the diarrhea
unless your doctor has told you to.
- Avoid exposure to sunlight or artificial UV
rays (sunlamps or tanning beds). It can make
your skin more sensitive to sunlight and
sunburn may result. Use a sunscreen (minimum
SPF 15) and wear protective clothing if you
must be out in the sun.
- Do not take iron supplements, multivitamins,
calcium supplements, antacids, or laxatives
within 2 hours before or after taking it.
Differin Gel
- It is used to treat acne.
- Apply once daily to affected areas after
washing & before going to bed for one month.
- Avoid contact w/ eyes, lips, angles of the
nose & mucous membranes.
- Do not apply to cuts, abrasions, eczematous
skin, severe acne.
- Minimise exposure to excessive sunlight,
including sunlamps. Take measures to protect
yourself from overexposure.
- Use it exactly as prescribed. Applying
excessive amounts or using the gel more than
once a day will not produce better results and
may cause severe redness, peeling, and
discomfort.
- If you have an allergic reaction or severe
irritation, stop using the medication and call
your doctor.
- In the first few weeks of treatment, your acne
may actually seem to get worse. This just
means that the medication is working on
hidden acne sores. Continue using the product.
It can take as much as 8 to 12 weeks before
you start to see improvement in your condition.
- Avoid using Differin with any other product
that can irritate the skin, such as medicated
soaps and cleansers, soaps and cosmetics that
have a strong drying effect, and products with
high concentrations of alcohol, astringents,
spices, and lime.

your eye, immediately flush with a large
amount of cool tap water.
- Be sure to use it for the full course of
treatment. If you do not, the medicine may not
clear up your infection completely. The
bacteria could also become less sensitive to
this or other medicines. This could make the
infection harder to treat in the future.
- Several weeks may pass before you see
improvement in your acne. Continue using
it for the full time recommended by your
doctor.
- Talk with your doctor before you use any other
medicines or cleansers on your skin.

- This drug may cause diarrhoea. If diarrhoea occurs, drink plenty of water to replace for thr fluid lost through diarrhoea. However, if you experience serious diarrhoea, consult your doctor immediately.
- Avoid taking alcohol while taking this drug.
- Avoid taking this medication if you are pregnant.
- Doxycycline may decreases the effectiveness of some oral contraceptives, another form of birth control should be used while taking this drug.
- If rashes occurs after taking this medication, stopped taking the drug and consult your doctor immediately.
Important counseling point for Differin gel
-This medication is for treatment of your acne problem.
- A thin film of Differin gel to be applied to area of the face affected by the acne every night at least one hour before bedtime for one month after washing your face.
- This medication may cause side effect such as redness of skin, dryness and itching.
- Avoid contact with eyes, lips, angles of the nose and mucous membranes. This medication should not be applied to cuts, abrasions, eczematous skin or sunburned skin.
- Exposure to sunlight including sunlamps should be minimized during the use of this medication. Use of sunscreen products and protective clothing over treated areas is recommended when exposure cannot be avoided.
- During the 1st 3 week while using this medication, your acne problem may seem to get worse. However, you should continue to use the medication unless irritation or other symptoms become severe.
- Full improvement of your acne condition can be seen within 12 weeks of usage.
- Wash your hands before and after using the cream.
Important counseling points for Clindamycin lotion
- This medication is for the treatment of your acne problem.
- Shake the solution well before each use. Clean and dry the affected area before applying this medication.
- A thin film of the topical solution twice daily to area of face that affected by acne for one month.
- This lotion works best if it is used at the same time each day.
- Use this lotion for the entire length of time that was prescribed even though your symptoms may improve sometimes after use.

When using PanOxy® acne gel, start with the lowest strength first, which is 2.5%.
Apply once daily onto affected area. Avoid contact with eyes, mouth, and mucous membrane. It may bleach the hair, clothes, and bed linens. Avoid exposure to excessive sunlight. Alcohol-based product, e.g. toner, cleanser, aftershave product may exacerbate stinging and burning sensation.
Side effect included: drying and peeling of skin, redness, transient stinging and burning.
Oxy Daily Wash is only for external use.
Directions of use: Wet hands, squeeze a little Oxy Daily Wash onto your hands and work into a lather. Gently smooth over your face and massage it in. Rinse thoroughly and dry gently. Avoid contact with the eye.
Use Oxy Daily Wash first, then apply PanOxy acne gel onto the affected area.
Other recommendations:
•Do not over-wash. Limit to two wash per day. Do not use harsh scrubs.
•Do not squeeze or pick the blemish. This may cause damage to the surrounding skin and most likely to leave a permanent scar. Too, will cause the bacteria deeper into the skin and cause inflammation and infection.

• Do not apply excessive amount as it may cause severe redness, peeling and discomfort.
• It can take as much as 8-12 weeks before you start to see improvement in your condition . So, continue using this product at all time.
Clindamycin lotion bd 1 month
• This medication is an antibiotic used to treat acne.
• Apply this lotion in a dabbing motion twice a day onto clean, dry affected area for a month.
• If you experience rashes, swelling of face, lips or eyelid, stop the medication and consult your doctor immediately.
• You may experience dryness, itchiness and oiliness when using this product.
• You must continue to use this product even if you see improvement.

- Remember to get ample sleep as its important for a healthy skin.
- Do not get too stress as stress will aggravate the acne condition. Engage in relaxing activity if needed.

ReplyDelete1. doxycyxline (doxymyxin-100)
- one capsule is to be taken once daily, every morning.
- Take this capsule after meal with a glass of water. Remain upright for at least 1/2 hour after taking the medication
(NOTE: the patient can sit down, not necessarily have to stand up for 1/2 hour)
- Do not take with dairy products as it may inhibit the drug getting absorbed into the body.
- Avoid direct and long exposure of sunlight as this medicine may cause your skin to be sensitive to light. Therefore, use sunscreen (or sunblock) if you need to go out of the house.
- Do not take this medication together with antacids, multivitamins, calcium pills, iron pills or laxatives that contain magnesium together with doxycycline as it may reduce the effectiveness of the antibiotics. If you must take them, take it 2 hours before or after taking doxycycline.
- Since this is an antibiotics, finish the whole course of medication

- You may experience burning, stinging sensation when using this medication.
- Redness is also common when using this medication.
- This medication may make your skin more sensitive to sunlight, therefore leading to sunburn. Hence, use sunscreen or sunblock when you are under direct or long exposure to sunlight.
- Do not apply this medication to open wounds (ie. cuts, burns, around the lips etc)
3. Clindamycin lotion (Dalacin T)
- Shake well before using the lotion.
- apply sufficient amount of lotion to the affected areas and its surrounding areas, two times a day.
- Clindamycin lotion works best if it is used at the same time each day.
- Be sure to use clindamycin lotion as directed by your doctor (period of usage) as the bacteria could get less sensitive to the medicine if you do not follow instructions.
- you may experience dryness or itching when using this medication.
- Do not get Clindamycin Lotion in your eyes or on the inside of your nose or mouth. If you accidentally get the medicine in your eye, immediately flush with a large amount of cool tap water.
